I, as parent/guardian of the above-named participant, understand that my daughter has to wear a face mask to participate in camp. I also understand everyone at camp (coaches, participants, spectators) has to wear a face mask. I, by typing "yes" understand as well that basketball is a contact sport and that injuries or illnesses can occur from participation in camp. I will not hold the camp organizers, volunteers, staff, or hosting facility liability for any injury or illness sustained from basketball camp activity.
